In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on record reviews and interviews, the facility failed to issue a written bed hold notice to a known family member or legal representative for 3 of 3 sampled residents who had been transferred to an acute care facility (Residents #30, #34, #8). Findings: 1. Documentation in Resident #30's clinical record indicated that he/she transferred to an acute care hospital on [DATE] and subsequently admitted . The clinical record contained no evidence that the facility issued a written bed hold notice to the resident, a family member, or legal representative upon transfer. On 12/17/19 at 10:30 p.m., in an interview with a Director of Nursing, a surveyor confirmed that Resident #30 or his/her representative did not receive a written bed hold policy for the hospital transfer on 11/9/19. 2. Documentation in Resident #34's clinical record indicated Resident #34 was transferred on 12/3/19 to an acute care facility for treatment of [REDACTED]. On 12/17/19 at 11:11 a.m., In an interview with the Director of Nursing, a surveyor confirmed that no bed hold notice was issued. 3. Documentation in Resident #8's clinical record indicated Resident #8's was discharged /transferred to an acute hospital on [DATE]. The clinical record lacked evidence that the facility issued a written bed hold notice to include cost of care to the resident and/or resident representative. On 12/16/19 at 2:00 p.m., in an interview with the Director of Nursing, a surveyor confirmed that lack of discharge/transfer notice to include appeal rights was given to the resident and/or resident representative. |
