Resident #3's record included a document which stated her end of life treatment wishes and was signed by Resident #3 and dated 12/5/87. The document stated Resident #3 did not want electrical or mechanical resuscitation if her heart stopped beating, naso-gastric tube feeding if she was unable to take nourishment by mouth, mechanical respirations if she was unable to sustain breathing, and if she was declared brain dead, she did not want mechanical means to prolong her life. Resident #3's current Physician order [REDACTED]. On 10/3/19 at 7:25 AM, the DON was asked about Resident #3's code status. The DON stated the facility was trying to figure it out.
