The facility has wall mounted single action fire alarm boxes that have a key hole. She was not sure how to use the wall mounted single action fire alarm box. She stated she would need to use a key to activate the wall mounted single action fire alarm box. On (MONTH) 19, 2013 CNA #16 was interviewed at 4:20PM regarding her response to a situational fire and the use of a fire extinguisher. The facility has wall mounted single action fire alarm boxes that have a key hole. She was not sure how to use the wall mounted single action fire alarm box. She stated she would need to use a key to activate the wall mounted single action fire alarm box. On (MONTH) 19, 2013 the Maintenance Supervisor as was interviewed at 4:35PM regarding the appropriate use of the wall mounted single action fire alarm boxes which have a key hole.
