In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This effected all residents receiving oral alimentation (59) Findings include: Observation of the kitchen on 7/8/13 at 11:45 a.m. revealed four (4) plastic bags of unknown yellow/orange food substance inside the walk in cooler. The four bags (4) bags had no label to identify the contents, date received and/or use by date. One bag in the refrigerator was opened and was not labeled identifying the date opened. Interview with the dietary supervisor on 7/8/13 at 11:55 a.m. revealed that the bag did not have an open date and/or the date the bag was received. Continued interview revealed that all food products should contain date received and/or open and use by date. 2015-06-01
