In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: 1. Review of the clinical record for resident #17 revealed Social Service Progress Notes dated 01/27/10 and 4/20/10 indicating that the resident signs out and ambulates around the community unsupervised. Review of the care plan for resident #17 revealed a care plan that addressed activities including that the resident had a strong preference to be out in the community. There were no approaches that specifically addressed this activity. Interview with a Certified Nurses Assistant on 9/21/10 at 4:05 p.m. revealed this resident leaves the facility and walks around in the community. 2. Review of the care plan for resident #15 revealed a care plan that addressed activities including that the resident goes out into the community. There were no approaches that specifically addressed this activity. Interview with the Administrator on 9/22/10 at 3:00 p.m. revealed that she was aware that these residents leave the facility to walk around the community. Continued interview revealed that they are required to sign in and out and both residents are compliant. Review of the sign in/out sheet revealed that both resident sign in and out when leaving the facility. 2015-06-01
