In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Review of the clinical record for resident #17 revealed a physician's orders [REDACTED]. Review of the Drug Regimen Review, performed by the consultant pharmacist, revealed that a gradual dose reduction was recommended on 6/29/09. The physician responded timely and ordered the medication to remain at the dose of 10 mg. each evening. There was no evidence that the consultant pharmacist had recommended a gradual dose reduction for this medication since 6/2009 as required. Interview with the Director of Nursing on 9/23/10 at 12:05 p.m. revealed the pharmacist had intended to review this resident's medications in August 2010 but did not. 2015-06-01
