In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: 1. Review of the clinical record for resident #8 revealed the resident was prescribed [MEDICATION NAME] 75 milligrams (mg.) and Aspirin 81 mg., both medication with blood thinning properties, each day for a [DIAGNOSES REDACTED]. There was no evidence that the physician was notified to determine if a change in treatment was required. Interview with the Director on Nursing on 09/22/10 at 11:05 a.m. revealed that there was no evidence that the physician was notified about the nose bleeds. 2. Review of the physician's orders [REDACTED]. Review of the April, 2010 Medication Administration Record [REDACTED]. There was no evidence that the physician had been notified that the medication had not been given in April. Interview with the DON on 9/22/10 at 3:40 pm revealed that there was no evidence that the physician had been notified that the [MEDICATION NAME] was not administered during the month of April, 2010. 2015-06-01
