In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: A tour of the dumpster area was conducted on 11/01/2017 at 11:08 a.m. revealed the following concerns: there was a pile of weathered garbage, containing paper trash and vinyl gloves, approximately three feet behind dumpster in a grass area; there was broken and damaged chairs stored behind the dumpster. Furthermore, three grease barrels are located beside the dumpster, and two of the three barrels had lids that were unsealed and ajar. On 11/2/2017 at 12:25 p.m. during an interview with the Corporate Dietary Manager, he stated that he was not aware the grease barrels were not able to be sealed. He stated that he would contact the company,[NAME]Industries, to order the locking rings that seal the barrels and lids. He also stated that the trash and debris around the dumpster was not the responsibility of the kitchen staff. On 11/3/2017 at 10:35 a.m., with Environmental Supervisor, stated that he orders a roll-off dumpster monthly to haul off the damaged furniture and equipment stored outside the dietary department. On 11/3/2017 at 11:04 a.m. during an interview with the Assistant Administration, he stated that there was no written policy or process related to the maintenance of the dumpster area. He stated that the Maintenance Department is responsible for the upkeep of the exterior grounds, including trash and debris around the dumpster. 2020-09-01
