In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The sample size was thirty-six (36) residents. Findings include: Review of resident # 164's clinical record revealed that they were readmitted to the facility after a hospitalized from [DATE]-03/05/12. Prior to the hospitalization and upon readmission, the resident had multiple pressure ulcers, weight loss, and a feeding tube. Review of the resident's current medical record kept at the nurse's station revealed that it only contained care plans for Impaired Breathing Pattern; Altered Cardiac Output; [MEDICAL CONDITION]; and Resuscitation status. When the overflow record was requested from the Medical Records department, it was noted to contain all of the other care plans, including pressure ulcers and nutrition. On 03/28/12 at 3:35 p.m., Licensed Practical Nurse (LPN) MDS (Minimum Data Set) Case Manager DD verified that resident # 164's care plans had been moved to the overflow chart, and since the resident was only gone seven days, all of their records should have been available on the current chart. 2015-11-01
