Findings include: 1. Review of the most recent quarterly Minimal Data Set ((MDS) dated [DATE] assessed resident # 39 as needing extensive assistance with personal hygiene that included nail care. Observation on 3/26/12 at 1:05 p.m., 3/27/12 at 8:40 a.m. and 11:23 a.m. and 3:27 p.m. revealed the resident had fingernails that were long and filled with a brown substance underneath. Interview with Registered Nurse (RN) Unit 200 Manager BB on 3/27/12 at 3:07 p.m. confirmed the resident's finger nails were dirty. 2. Review of the most recent MDS assessment dated [DATE] for resident # 31 documented the resident was totally dependent on staff for hygiene that included nail care. During observation of the resident on 3/26/12 at 1:09 p.m., 3/27/12 at 8:44 a.m., 11:25 a.m. and 3:10 p.m. revealed the resident had long fingernails with brown substance underneath. Interview with the RN Unit Manager BB: on Unit 2 on 3/27/12 at 3:15 p.m. confirmed the resident's finger nails needed to be cleaned. 3. Review of the most recent MDS assessment dated [DATE] assessed resident # 90 as needing total assistance with personal hygiene. Observation on 03/26/12 at 2:27 p.m., 3/27/12 at 8:10 a.m., 12:01 p.m. and again at 3:12 p.m. revealed the resident to be in bed with long fingernails and with a brown substance underneath them. Interview with the RN Unit 200 Manager on 3/27/12 at 3:15 p.m. confirmed the resident's finger nails needed to be cleaned. | 2015-11-01 |
