In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

8677 SIGNATURE HEALTHCARE OF SAVANNAH 115120 815 EAST 63 STREET SAVANNAH GA 31405 2012-03-29 280 D 0 1 G6KB11 **NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on record review, observation and staff interview the facility failed to ensure that the care plan related to Activities of Daily Living (ADL) for one (1) resident (# 31) on a sample of thirty-six (36) residents was updated to reflect the actual care the resident was assessed for and receiving. Findings include: Review of the most recent Minimal Data Set (MDS) assessment dated [DATE], as well as the previous annual MDS assessment dated [DATE] assessed resident #31 as being totally dependent on the staff for hygiene, tolieting, and eating. Review of the care plan for self care deficit for hygiene, tolieting, eating and bathing documented the resident needed only set up for grooming,hygiene and bathing supplies and was to be encouraged to complete task with assistance as needed. This care plan also documented that the resident only needed set up for the meal tray such as to cut meat, butter bread and remove wrappers. Observation of the resident during the breakfast meal on 3/27/11 at 8:40 a.m. and again on 3/28/11 at 8:26 a.m. revealed the resident was in the day room being fed by a Certified Nursing Assistant (CNA). The resident was in a wheelchair and had a contracture to the right hand which was in a splint. The resident also had limited use of the left hand. Interview with a CNA CC staff on 3/28/12 at 8:26 a.m. revealed the resident was totally fed by staff and had been for some time. This CNA also revealed the resident was dependent on staff for all tolieting and hygiene required as she was incapable of doing for her/himself. Interview with the MDS Nurse Licensed Practical Nurse DD on 3/28/12 at 2:38 p.m. revealed the care plan for ADL's did not reflect the resident at all as the resident required total dependence of all for ADL's. 2015-11-01
