In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Sample size was 35 residents. Findings include: Resident #46 was admitted to the facility on [DATE], with [DIAGNOSES REDACTED]. A Minimum Data Set (MDS) annual reassessment dated [DATE] was completed by staff. Review of this MDS assessment, including the accompanying Care Area Assessments (CAA's), revealed the facility staff failed to accurately assess this residents dental status. During interview on 11/3/2017 at 4:47 p.m., MDS nurse AA confirmed that R #46 was not accurately coded on the Annual Reassessment MDS or included in the CA[NAME] She stated that reassessments are done by face to face visits. She further stated that she may be confusing this resident with another. If she is made aware of a miscoding by staff or herself, then she will make a modification. She could not find a dental assessment on his record.	2020-09-01
