ADEL GA 31620 2012-02-23 428 D 0 1 028H11 Based on record review and staff interview, it was determined that the facility failed to act upon a pharmacy recommendation for one resident (#86) from a total sample of 32 residents. Findings include: Resident #86 had a recommendation from the pharmacist on 9/30/11 for the physician to review the continued need for the medication Procrit due to the medication being held several times for hemaglobin levels greater than 12. The resident's attending physician's documented response on the recommendation form was that another physician had ordered the medication. There was no further evidence in the clinical record that the pharmacist's recommedation was addressed.
