Findings include: Observation of the shared bathroom for room #102 and #104 on 2/20/12 and 2/23/12 at 11:45 a.m. revealed foul urine odor. During an interview on 2/23/12 at 11:45 a.m., a housekeeping staff said that urine was in the tile floor and grout. He/She said the room had always had that odor. | 2016-03-01 |
