In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: UNIT 1: Review of facility records revealed no evidence that the required Medicare liability and appeal notice was provided to resident #83 prior to being discharged from Medicare Part A services. Interview on 8/28/13 at 8:20 a.m. with the Director of Nurses (DON) revealed that the resident did not receive a letter of appeal because he/she used all of the one-hundred (100) days allotted. Continued interview revealed that the DON did not think we had to issue those letters unless there was a requested demand bill. 2017-10-01
