In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Record review for resident #68 revealed the resident had a [DIAGNOSES REDACTED]. The 9/25/13 laboratory test results revealed that the resident's [MEDICATION NAME] level was low at 7.4, with an acceptable range of 10-20. On 10/1/13 the physician wrote an order to increase the resident's [MEDICATION NAME] to 400 mg for two days and then to recheck the [MEDICATION NAME] level in two weeks.
