In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

6493 CORDELE HEALTH AND REHABILITATION 115429 1106 NORTH 4TH STREET CORDELE GA 31015 2014-02-13 312 D 0 1 01DX11 **NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observations, record review and staff interviews the facility failed to ensure that (1) resident ( E) who required assistance with grooming, was observed with her hair uncombed for two of four (2/4) days of the survey from a sample size of twenty-eight (28) residents. Findings include: Review of the clinical record for resident E reveals the resident is a ninety-one (91) year old African American resident admitted to the facility since 2012. She is alert, pleasantly confused with a Brief Interview (BIMS) score of five (5). Review of the Quarterly Minimum Data Set (MDS) assessment dated [DATE] revealed that the resident was assessed as being always continent of bowels and bladder. She required limited assistance of one (1) person with dressing, personal hygiene, and physical help with bathing. Continued review revealed that the resident was independent with bed mobility, transferring, ambulating, locomotion, eating and toileting. Review of the resident's care plan revealed there was a care plan dated 07/31/12 for requiring minimal assistance with activities of daily living (ADLs) related to weakness in her lower extremities, and impaired cognition. Noted on 09/02/13 that the resident refuses mouth and nail care at times. The interventions included to assist daily with grooming, dressing, and oral care, and to encourage the resident to participate in ADLs to her ability. Review of the Resident Bath List revealed that resident E receives a bath every Monday, Wednesday, and Friday. Observation of resident E on 02/10/14 at 1:00 p.m. ambulating independently in the hallway. She was appropriately dressed, her hair was disheveled with braids that were uncombed. The resident was pleasantly confused and ambulated the hallway holding a towel. Observation of resident E on 02/11/14 at 10:55 a.m. ambulating from her room to the dining room for morning exercises with Certified Nursing Assistant (CNA) CC. She was appropriately dressed, her hair was observed to be uncombed, and sticking up with her braids coming loose. During a family Interview on 02/11/14 at 11:48 a.m., resident E family member revealed that he had spoken with some of the CNAs, and the Social Worker about his mother's hair being uncombed. He further revealed that he visits his mother two (2) to three (3) times a week, and quite a few times her hair was uncombed. Continued interview revealed that the staff was asked to comb her hair in big braids to make it easier to take care of. He further revealed that he bought a big container of hair grease, and it seems like they use it on everyone else but his mother because they don't comb her hair that often. He revealed that he cannot afford to leave money for the beauty shop, but that he expected that his mother's hair should still be washed and combed by the staff. Observation of resident E on 02/12/14 at 9:00 a.m. revealed the resident standing in the doorway of her room. She was appropriately dressed and groomed, her hair was neat, and freshly braided. The resident revealed that she had a shower this morning. An interview with CNA AAon 02/12/14 at 1:50 p.m. revealed that she works about once a week with resident E, and that their assignments change from working on the floor, to being assigned to the bath team. CNA AA revealed that she gave resident E a bath today, and braided her hair and revealed that the resident is able to bathe, and dry herself, but she needs assistance with reaching her back. CNA AA further revealed that sometimes the resident will refuse to have her hair washed because it's too cold, but the resident likes to have her hair braided. Continued interview with CNA AA revealed that all the CNAs help to braid hair for the African American residents, because the beauty shop doesn't really know how to do their hair. CNA AA revealed that resident E can unbraid her own hair. An interview with CNA BB on 02/12/14 at 2:00 p.m. revealed that she takes care of resident E once or twice a week, and she did not work on Monday and Tuesday. CNA BB revealed that they wash the resident's hair on her bath day, and blow dry, and braid it up. If her hair is sticking up on her head, someone will try and braid it for her. CNA BB revealed that the resident's son does not like for the CNAs to braid the resident's hair in small tiny braids. He wanted his mother to have big braids that she can take out by herself. An interview with CNA CC on 02/12/13 at 1:15 p.m. revealed that she took care of resident E on Tuesday, but did not comb her hair. An interview with the Director of Nursing (DON) on 02/12/14 at 2:25 p.m. revealed that resident E family had a preference about the way her hair was combed, some of the other African American residents had their hair braided in smaller braids. The DON further revealed that there might have been times where the resident attempted to take out the braids herself. The DON further revealed that hair care goes along with the residents baths, and that any CNA can fix the residents hair. Continued interview revealed that it was her expectation that all residents were appropriately dressed, and groomed daily ,and their hair combed daily by staff. An interview with Registered Nurse (RN) DD on 02/12/13 at 3:00 p.m. revealed that the facility did not have a form for documenting that ADLs, grooming, and hygiene was completed by the CNAs. R.N. DD further revealed that the CNA signs her name on the Bath List next to the resident's name, and that is how they document that a resident was given a bath on that day. 2017-12-01
