In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: During an interview with the Social Services Director (SSD), on 05/21/15 at 11:35 a.m. the SSD revealed that resident #50 was discharged from skilled services on 05/19/15 and that he remained in the facility afterwards. Review of Notice of Medicare Non-Coverage notice dated 05/13/15 revealed that resident #50 signed the CMS Form but was not provided the CMS Form providing the resident with an informed choice regarding whether or not to continue with services and the potential costs. The SSD acknowledged that she did not have resident #50 sign the Advanced Beneficiary Notice (ABN) because she had never seen the notice before and was unaware that she was required to provide the notice to residents being discharged from Medicare Part A who were to remain in the facility. 2018-11-01
