In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: An observation conducted on 7/22/15 at 12:36 p.m. with Dietary Aide DD revealed that the fried chicken on the steam table was not at the appropriate temperature. The top pieces of chicken measured one hundred degrees Fahrenheit (100 F)and the bottom pieces of chicken measured one hundred and twenty-five (125 F). An interview conducted on 7/22/15 at 12:39 p.m. with DD revealed that the food was delivered from the hospital and placed on the steam table. Temperatures are checked upon arrival from the hospital but at no other time during serving. DD further revealed that they do not have reheating capabilities in the satellite kitchen and food has to be sent back to the hospital to be reheated. An interview conducted on 7/23/15 at 9:00 a.m. with the Director of Nursing (DON) revealed that after investigation, it was found, the left side of the steam table had not been turned on. 2019-08-01
