In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Review of the clinical record revealed R#800 was admitted to the facility on [DATE]. It was documented in the clinical record that the patient/caregiver refused the flu vaccine. Further review of the clinical record revealed no documented evidence that the resident received education/information prior to being offered the flu vaccine. Review of the clinical record revealed R#801 was admitted to the facility on [DATE]. It was documented in the clinical record that the patient/caregiver refused the flu vaccine. Review of the clinical record revealed R#112 was admitted to the facility on [DATE]. It was documented in the clinical record that the patient/caregiver refused the pneumonia vaccine. Further review of the clinical record revealed no documented evidence that the resident received education/information prior to being offered the pneumonia vaccine. Review of the clinical record revealed R#113 was admitted to the facility on [DATE]. It was documented in the clinical record that the patient/caregiver refused the pneumonia vaccine. Further review of the clinical record revealed no documented evidence that the resident received education/information prior to being offered the pneumonia vaccine. Review of the clinical record revealed R#114 was admitted to the facility on [DATE]. It was documented in the clinical record that the patient/caregiver refused the pneumonia vaccine. Further review of the clinical record revealed no documented evidence that the resident received education/information prior to being offered the pneumonia vaccine. During an interview on 6/4/19 at 3:43 p.m., the Administrator and the Clinical Unit Manager AA both indicated that the influenza vaccine is only offered during the flu season and that information/education was only provided at that time. They confirmed that there was no documentation to indicate information/education was provided to the residents or their representatives for either vaccine at the time of the influenza or pneumococcal vaccine offering. During an interview on 6/5/19 at 11:35 a.m., the Director of Nursing (DON) and the Infection Control Registered Nurse (RN) BB both stated that there are no procedures/protocols for the pneumonia vaccine for this unit. If the resident asks for information they will provide it to them. The DON also stated that she could not provide information about the influenza vaccines for the residents already discharged because the computer locks them out of that section unless it is within the flu season. Review of the facility policy titled Standing Orders for Influenza Vaccine Protocol dated 10/19/18 revealed: The standing order protocol will be in effect during the CDC's recommended time-frame (usually (MONTH) 1 through (MONTH) 31). D. Provide Vaccine Information Statements: Provide all patients with a copy of the most current Vaccination Information Statement.
