In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Refer F329 Finding include: Review of the physician orders [REDACTED]. Review of section N of the quarterly Minimum Data Set (MDS) assessment dated [DATE] revealed antipsychotic medications were documented as administered each day of the seven day look back period. Further review revealed the comprehensive care plan dated revised 12/1/16 indicated there was not a care plan developed to address the use of the antipsychotic medication. During an interview on 2/9/17 at 6:40 p.m., the MDS Coordinator confirmed the care plan was not revised to include [MEDICATION NAME] after the quarterly MDS was conducted in (MONTH) (YEAR). 2020-09-01
