In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Record review of the chart revealed an order, with a start date of (MONTH) 1, (YEAR), for [MEDICATION NAME] 0.25 milligrams (mg) with one tablet taken by mouth twice daily as needed for 60 days for agitation. Review of the Medication Administration Record [REDACTED]. Interview on 1/24/19 at 9:40 a.m. with Licensed Practical Nurse (LPN) II confirmed that the [MEDICATION NAME] PRN order started in (MONTH) and should have ended (MONTH) 1, 2019. It was reported that typically orders on the rehab unit are not written this way and that the pharmacy notifies when there is a hard stop on an order. Interview with the Registered Nurse (RN) Unit Manager JJ on 1/24/19 at 9:45 a.m. revealed that she had received a report from the pharmacy consultant on Tuesday but she had not reviewed or followed up on the recommendations yet. 2020-09-01
