In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Record review for Resident #1 revealed a 10/07/2010 physician's orders [REDACTED]. During a telephone interview conducted on 11/01/2010 at 11:30 a.m. with Staff Member "AA", this staff member stated that upon review after the resident's hospital transfer, it was determined that the facility could not meet the resident's needs and the decision was made to not readmit the resident. However, further record review revealed no evidence to indicate that the resident and the family member/legal representative were notified, in writing, of the transfer/discharge, the reasons for the transfer/discharge, the effective date of the transfer/discharge, the location to which the resident was transferred/discharged , a statement that the resident had the right to appeal the action to the State, and the name, address and telephone number of the State long term care ombudsman. During an interview with Administrative Staff "BB" conducted on 12/13/2010 at 9:45 a.m., this staff member acknowledged that a transfer/discharge notice letter had not been sent. 2014-04-01
