R#91 was observed needing assistance with changing out of soiled clothing on more than one occasion. Findings include: According to the Admission Record R#91 was re-admitted to the facility on [DATE]. Review of the [DIAGNOSES REDACTED]. Review of the most recent quarterly Minimal Data Set ((MDS) dated [DATE] revealed under Section B0700 Makes Self Understood and B0800 Ability to Understand Others R#91 was understood by others and able to comprehend others. Review of Section C - Cognitive Patterns R#91 scored an 8/15 on the Brief Interview for Mental Status (BIMS) indicating R#91 was cognitively impaired. Section G - Functional Status G0110 documented that R#91 required extensive assistance of one person for dressing, supervision and setup for eating. Review of the Care Plan initiated on 11/16/16 for R#91 had the following focus, The resident has an ADL (activities of daily living) performance deficit r/t (related to) [MEDICAL CONDITION], limited Mobility, Stroke (sic). The goal was the resident's ADL's will be met with dignity and respect and be maintained through next review. The interventions related to dressing were, allow sufficient time for dressing and undressing, assist the resident in selecting simple comfortable clothing to enhance his ability to dress himself and resident requires assistance by staff to dress. During an observation on 8/7/17 at 3:31 p.m., R#91 was observed in bed wearing a green short sleeved shirt. During an observation on 8/8/17 at 10:59 a.m., R#91 was observed in bed wearing the same green short sleeved shirt. There was food debris on the upper left shoulder. During an observation on 8/9/17 at 12:32 p.m., R#91 was observed sitting in his wheelchair in his room. R#91 was wearing the same green short sleeved shirt with stains noted to the upper left shoulder, he also had on grey sweat pants. During an observation on 8/9/17 at 1:55 p.m., R#91 was observed in the hallway near the Director of Nursing (DON) office. He was wearing the same clothing but had a piece of food on his left shoulder. During an observation on 8/9/17 at 3:00 p.m., R#91 was observed in the hallway near the DON office. He was wearing the same clothing but had a piece of food on his left shoulder. During an interview with the North Unit Manager Licensed Practical Nurse (NUMLPN) FF on 8/9/17 at 3:36 p.m., she indicated residents should be neatly groomed, teeth brushed and their clothing should be clean. She further indicated after a meal staff should be changing the resident's clothing if they spilled food, there clothing should be changed as soon as possible but no longer than 30-45 minutes. During an observation and interview on 8/9/17 at 4:34 p.m., R#91 was observed sitting in the common area in his wheelchair. R#91 had the same shirt and pants on. The piece of food remained on his left shoulder, there were more stains than before, and his pants were soiled on his left thigh. When asked why he had been wearing the same shirt for the last three days, he stated, I want to wear short sleeves and only have this one shirt. He was informed his shirt was dirty and he had a piece of food on his shoulder from after lunch. The resident turned to his left and stated, Oh I did not see that, I don't like having dirty clothing, if I had known I would have changed. Resident gave permission to look in his closet. Hanging in R#91's closet revealed there were other pants and at least two white short sleeved shirts for him to wear. During an observation on 8/9/17 at 5:56 p.m. R#91 was observed in the common area, his clothes were still soiled. During an interview with Certified Nursing Assistant (CNA) OO on 8/10/17 at 8:33 a.m., she indicated if a resident's clothing was soiled after a meal they were to change their clothing as soon as possible. During an interview on 8/10/17 at 9:28 a.m., CNA QQ revealed they will clean residents up as much as they can in the dining room. If their clothes are soiled they will take them to their room and change their clothing. When asked if the resident wanted to wear short sleeves and only had one shirt what would she do, she stated, I would explain to them they only had long sleeve shirts and then change their shirt in they agreed. She further revealed if they did not want to change she would notify the nurse. When asked if a resident's clothing was dirty after lunch and still dirty after dinner was that ok, she shook her head no and stated, We just do not do that here. | 2020-09-01 |
