In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

For R175, the facility failed to provide her with feeding assistance for 21 minutes while her 2 tablemates ate their meals. For R117, the facility failed to serve her meal at the same time as her 3 tablemates. Findings include: 1. An observation on 7/11/17 at 11:32 AM in the Elsmere dining room revealed that a table of 3 residents (R52, R162 and R175) were served their meals. R52 and R162 were observed immediately eating on their own. From 11:32 AM to 11:53 AM, R175 was observed with her meal in front of her untouched until feeding assistance was provided. R175 sat unassisted for 21 minutes with her meal in front of her while her 2 tablemates ate their meals. 2. An observation on 7/11/17 at 11:40 AM in the Elsmere dining room revealed only 3 out of 4 (R29, R39, R93 and R117) residents seated at the table were served their meals at the same time. R117 watched her 3 tablemates eat their meals for approximately 11 minutes until she was served her meal at 11:51 AM. Findings were reviewed with E2 (DON) and E3 (RN/Staff Ed) on 7/19/17 at 3 PM.
