E5 (beautician), was not able to identify potential abuse and did not know how to handle reports of resident abuse, including whom to report to. Findings include: Review of the facility policy, dated (MONTH) (YEAR), and entitled Abuse, Neglect, Mistreatment, Misappropriation and Exploitation stated, All employees, contracted providers, and volunteers upon hire, and annually thereafter, will receive mandatory training on issues related to abuse, neglect, mistreatment, misappropriation of resident property and exploitation, consistent with their expected roles, pursuant to the Training Policy. On 9/7/17 at 11:14 AM, E5 (contracted beautician) was interviewed regarding reporting observed or suspected abuse or mistreatment. E5 stated that she had worked at the facility for about one year. E5 stated that she had never observed any mistreatment of [REDACTED]. On 9/7/17 at approximately 3:30 PM, findings were discussed with E2 (DON) and E4 (ADON). On 9/7/17 at 3:57 PM E2 reported that E5 was not trained on issues related to abuse, neglect, mistreatment, misappropriation of resident property and exploitation.
