In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Since maximal effect of a given dose is seen within 4 weeks, periodic lipid determinations should be performed at this time and dosage adjusted according to the patient's response to therapy and established treatment guidelines . Review of R126's pharmacy reviews from (MONTH) (YEAR) to (MONTH) (YEAR) revealed the consultant pharmacist's failure to identify that R126 was not getting lipid determinations performed periodically as indicated while taking Pravastatin. In an interview on 7/14/16 at 11:15 AM, E5 (LPN, UM) confirmed the finding. Findings were reviewed with E2 (DON) on 7/18/16 at 10:10 AM. Cross refer to F329, example 2 2.
