In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: Review of R47's clinical record revealed: 3/13/19 - R47 was admitted to the facility for skilled nursing and rehabilitation. 4/16/19 - A physician's orders [REDACTED]. Review of R47's clinical record lacked evidence that the facility provided the following information to the receiving health care provider: - lab results dated 4/16/19; - an accurate ADL status of R47 and current vital signs on the interagency nursing communication record; - updated comprehensive care plan; - special instructions/precautions for ongoing care, including adaptive equipment needs; and - a copy of the resident's discharge summary. 7/9/19 at 11:38 AM - During an interview, E17 (SW) stated that he/she did not send R47's care plan to the receiving provider. 7/15/19 at 12:30 PM - Findings were reviewed during the Exit Conference with E1 (NHA), E2 (former DON), E3 (acting DON), E6 (NP) and E16 (ED).
