Findings included . During observations on the second and third-floor nursing units on (MONTH) 11, (YEAR), between 11:15 AM and 2:35 PM, call bells in two (2) of 34 resident rooms (#210 and #330), did not alarm when activated. Also, the pull cord from the call bell located in the bathroom of resident room [ROOM NUMBER] one (1) of 34 resident rooms was missing. A breakdown in the communication system could prevent or delay the resident, staff or the public from alerting staff.
