In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

During an observation of the resident on (MONTH) 10, (YEAR), at approximately 11:49 AM, one bag of [MEDICATION NAME] 1.5 enteral feed was hanging and delivering product to the resident. The resident's name, the date, time and rate of feeding was not recorded on the label affixed to the bag. Employee #18 who was present at the time of the observation acknowledged the finding and stated, Since we don't know the date and time it (the bag of enteral feed) was hung, I will replace it. Facility staff failed to record the date and time on the enteral feeding bag so that staff may ensure the feeding hung has not exceeded the 48-hour hang time. 2020-09-01
