In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on observations, medical record review and interview, for one (1) of 56 sampled residents the facility failed to provide an environment free from accident hazards as evidenced by several remote bed controllers cords that were frayed in six (6) of 34 resident rooms. Resident#158. Findings included . During observations throughout the facility on (MONTH) 11, (YEAR), between 11:15 AM and 3:00 PM, remote bed controller electrical cords were frayed in resident room [ROOM NUMBER] (A and B beds), 131, 222B, 229A, 313 (A and B beds), 322A, six (6) of 34 resident rooms surveyed. The uncovered, exposed electrical wires created a potential electrical shock hazard to residents, staff and the public. During a face-to-face interview on (MONTH) 11, (YEAR), Employee #12 acknowledged these findings at the time of observation. |
