In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #7. Findings included . A review of the Pre-Admission Screening/Resident Review for Mental Illness and or Mental [MEDICAL CONDITION] Level I (PASRR) screen, signed as completed by the facility staff on (MONTH) 31, 2014, revealed that Resident #7 was identified as positive for major mental disorder [MEDICAL CONDITION], and a Level II screen is required. There is no evidence that the facility staff completed the Level II Pre-Admission Screening/Resident Review as indicated from the level I screening. Facility staff failed to ensure that the Level 2 Pre-Admission Screen/Resident Review for Mental Illness and or Mental [MEDICAL CONDITION] for Resident #7 who had a [DIAGNOSES REDACTED]. A face-to-face interview was conducted with Employee #11 (SW) on 7/25/2019 at 9:00 AM. After a review of the findings she acknowledged that the level II screening was not done. 2020-09-01
