In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

**NOTE- TERMS IN BRACKETS HAVE BEEN EDITED TO PROTECT CONFIDENTIALITY** Based on medical record review and staff interview for one (1) of 56 sampled residents, facility staff failed to accurately code the Comprehensive Minimum Data Set (MDS) for one (1) resident with a [DIAGNOSES REDACTED]. Findings included . Resident #175 was admitted on [DATE] with [DIAGNOSES REDACTED]. Review of the Comprehensive Minimum Data Set ((MDS) dated [DATE] showed Resident #175 is cognitively intact as evidenced by a Brief Interview for Mental Status score of 15. Review of the physician's orders [REDACTED]. Further review of the MDS showed Section I Active Diagnoses: [REDACTED]. Facility staff failed to accurately code the MDS to include resident's active [DIAGNOSES REDACTED]. During a face-to-face interview on 7/29/19 at 11:30 AM Employee #12 acknowledged the finding and stated yes, the resident has [MEDICAL CONDITION] I will make the change now. |
