In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #12 The findings include: 1. Facility staff failed to accurately reconcile a controlled substance medication for Resident #12 receiving morphine. On December 28, 2010 at approximately 11:00 AM one (1) of one (1) bottle of Morphine Concentrate was observed with 20 mls of medication remaining in the bottle. A review of the " Controlled Drug Receipt/Record/Disposition Form " revealed that on December 28, 2010 at 9:00 AM 0.25mls of morphine concentrate was administered to the resident and the amount remaining was 15.50 mls. There was no evidence that facility staff accurately reconciled the amount of Morphine Concentrate that remained in the bottle with the disposition form. This observation was made in the presence of Employee # 7 who acknowledged the aforementioned findings. 2. Facility staff failed to discard one (1) of one (1) vial of Humalog stored beyond the expiration date. On December 28, 2010 at approximately 10:45 AM one (1) of one (1) vial of Humalog 100 was observed stored in the 4th floor medication refrigerator. The open date was documented as November 20, 2010 with an expiration date of December 28, 2010. The vial of Humalog was available for use to be administered to the resident for 38 days, eight (8) days past the expiration date. This observation was made in the presence of Employee # 7 who acknowledged the aforementioned findings. 2014-02-01
