In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings included . A review of the Shift count Narcotic records on Unit 1 Green was completed on (MONTH) 19, 2019, at approximately 9:00 AM. The review showed that on (MONTH) 5, 2019, the Shift count Narcotic was missing a nurse signature (indicating it was not done) in the space allotted the nurse going off duty to reconcile the Narcotics for the 7:30 AM to 3:30 PM shift. A review of the Shift count Narcotic records on Unit 1 Orange was completed on (MONTH) 19, 2019, at approximately 9:10 AM. On (MONTH) 12, 2019, the Narcotic count sheet, showed the spaces allotted for nurse signature going off duty to reconcile the Narcotics for the 11:00 PM to 7:30 AM shift was left blank indicating Not Done. A review of the Shift Verification of Accuracy of Controlled Drug Record to the Actual Narcotic Count (Reconciliation Controlled Drug Count Verification Form) directed, Shift count sheet for Narcotics balance must be verified by the nurse coming on duty and nurse going off duty at each change of shift The evidence showed that the system's use for acceptable standard of practice to account for the receipt, usage, disposition, and reconciliation of controlled medications was not followed.
