In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This failed practice created a potential for incomplete and/or inaccurate care planning. Findings: Record review on 1/7-8/19 revealed Resident #1 was admitted to the facility with a [DIAGNOSES REDACTED]. Review of Resident #1's MDS assessments revealed the following: an admission assessment dated [DATE], a quarterly assessment dated [DATE], a discharge assessment with return anticipated dated 12/24/18 and an Entry tracking record dated 12/28/18. A quarterly assessment, due by 9/13/18, was missing. During an interview on 1/8/19 at 11:56 am, MDS Coordinator (MDSC) #2 stated he/she had not done the quarterly assessment. During an interview on 1/9/19 at 2:13 pm, MDSC #1 stated there are 3 MDS coordinators. Each coordinator is responsible for their assigned residents. A hand written calendar is used to document when MDS assessments are due for each resident. 2020-09-01
