In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This failed practice placed all residents at risk for less than optimal care or lack of adequate care to meet their individual needs. Findings: Record review on 4/26/18, revealed Resident #1's MDS assessment dated [DATE] was not transmitted. Record review on 4/26/18, revealed Resident #3's MDS assessment dated [DATE] was not transmitted. Record review on 4/26/18, revealed Resident #5's MDS assessment dated [DATE] was not transmitted. An interview on 4/26/18 at 2:37 pm, Licensed Nurse (LN) # 3 stated no staff in the facility have transmittal ability/access for sending MDS information as required. The LN confirmed the MDS information has not been submitted for any resident. During an interview on 4/26/18 at 2:50 pm, with the Chief Nursing Officer confirmed no facility staff have the ability to transmit the MDS data as required. 2020-09-01
