Resident identifier: #21. Facility census: 58. Findings include: a) Resident #21 On 07/21/11 at approximately 2:00 p.m., the medical record for Resident #21 revealed a physician's orders [REDACTED].) The order, written at 10:00 p.m. on 07/15/11, stated: (symbol for 'change') [MEDICATION NAME] to 66 ml BID (twice per day). The Medication Administration Record [REDACTED]. On 07/22/11 at approximately 3:00 p.m., Employee #76 (registered nurse / nurse practice educator) and Employee #77 (a corporate registered nurse) provided a copy of the label attached to the bottle of [MEDICATION NAME]. The label contained the resident's name as well as the date 07/14/11. The label indicated the following dosage instructions: 22.5 ml by mouth every eight (8) hours. The bottle also identified the contents of the medication as [MEDICATION NAME] 10 gm/15 ml solution. The West Virginia Board of Pharmacy, West Virginia Code of State Rules, states: 19.13.2. To dispense, deliver, or distribute a prescription drug order accurately as prescribed. For the purposes of this paragraph 'accurately as prescribed' means a. To the correct patient (or agent of the patient) for whom the drug or devise was prescribed; b. with the correct drug in the correct strength, quantity, and dosage from ordered by the practitioner; a pharmacist may substitute a generic drug pursuant to W.Va. Code? 30-5-12b; and c. With correct labeling (including directions for use) as ordered by the practitioner. On 07/22/11 at approximately 3:30 p.m., Employees #76 and #77 confirmed the label on the bottle of [MEDICATION NAME] did not accurately reflect the current order by the prescribing practitioner. | 2016-01-01 |
