This affected nine (9) resident rooms in the facility. Room numbers: 99, 110, 111, 112, 113, 221, 224, 226 and 227. Census: 64. Findings include: a) Resident room #s 99, 110, 111, 112, 113, 221, 224, 226 and 227 Observations of the facility, during Stage I of the quality indicator survey process, revealed doors and walls that were scratched, marred, and/or scuffed. Door frames to bathrooms and to the hallways had paint chipped off of the door frames. This was discussed with the maintenance director, Employee #63, as part of the environmental component of the survey process.
