In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

This was true for four (4) of nine (9) residents. Resident identifiers: Resident #1, #9, #6, #15. Facility census sixteen (16). Findings include: a.) During lunch on 12/3/12, two (2) residents were observed with pureed meals with food white in color and lacking any accent color with the meal. Resident identifiers #6 and #15. b.) When Breakfast was served to Resident #1 on 12/4/12 it was all white and shades of beige. c.) The lunch meal received by Residents #1 and #9 on 12/4/12 was also white and shades of beige. d.) On 12/5/12 Resident #9 was served a lunch that was all white and lacked any accent color. e.) The appearance of the meals was discussed with the DON at 3:00 p.m. on 12/5/12.
