The facility did not post the address for the State survey agency. This agency serves as the entity to which residents can make formal complaints about the care they are receiving. This practice had the potential to affect all residents and/or their responsible parties. Facility census: 118. Findings include: a) On 09/16/13 at 4:00 p.m., the resident council president (Resident #61) said she did not know where the posting was that had the name, address, and telephone number of the State survey agency. This agency is responsible for receiving complaints from residents regarding their care in nursing homes. At 4:30 p.m. on 09/16/13, an observation of the posting revealed the name and telephone number of the State survey and certification agency was posted; however, the posting did not contain the address for this agency. On 09/17/13 at 10:55 a.m., during an interview, the administrator (Employee #66) said he said he would update the postings so they contained this information. | 2017-08-01 |
