In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The heater was not functioning correctly in Resident #120's bathroom. Facility census: 105. Findings include: a) Resident #120 An interview was conducted on 04/10/14 at 10:10 a.m. with Resident #120. During this Stage 1 interview, the resident stated his room was cold when staff left the bathroom door open. He said leaving the bathroom door open created a draft, causing his bedroom to be cold. An observation of Resident #120's room was conducted with the maintenance director, Employee #124, on 04/10/14 at 10:30 a.m. When the situation was discussed, Employee #124 stated if the resident was cold, the resident could just turn the heater on in the bathroom. When he first entered the bathroom, the maintenance director commented the bathroom temperature was a little bit cooler than the bedroom temperature. Employee #124 checked the air temperature in the bathroom. It was 66.5 degrees Fahrenheit. He then attempted to turn on the heater in the bathroom, but the heater would not function. Employee #124 stated to the resident, I will have the heater fixed. 2018-04-01
