In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Drinking glasses were not adequately air dried to prevent moisture from being trapped inside the glasses. This created a potential for the growth of microorganisms. This practice had the potential to affect all residents who received meals from the kitchen. Facility census: 66. Findings include: a) During the initial tour of the dietary department, beginning at 11:23 a.m. on 11/04/13, the following sanitation infraction was observed with the dietary manager, Employee #41. 1) Observation of a small storage cabinet, mounted on the wall of the kitchen, at 11:30 a.m. on 11/04/13, found drinking glasses had been stacked together before air drying. There was moisture trapped between the glasses. When the glasses were picked up, rings of water were visible on the shelf. The dietary manager stated she would wash the glasses again. 2018-04-01
