In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Findings include: a) Resident #66 This resident's medical record was reviewed on 01/22/14 at 12:16 p.m., and revealed the resident's history of pressure ulcers since admission. The quarterly minimum data set (MDS) assessment, with an assessment reference date (ARD) of 09/18/13, was coded 1 under Item M0210, which indicates the presence of one or more unhealed pressure ulcers at Stage 1 or higher. Item M0900, was coded 1, which indicated pressure ulcers were present on the prior assessment. The annual MDS assessment, with an ARD of 12/18/13, was coded 0, under Item M0900, which indicated pressure ulcers were not present on the prior assessment. An interview conducted with Employee #102 (Registered Nurse Assessment Coordinator), on 01/22/14 at 4:20 p.m., confirmed the MDS assessment was coded incorrectly. 2018-04-01
