In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident #159 tearfully stated she was bored and had nothing to do except walk the hall or lie in bed and watch television (TV). This resulted in actual harm to Resident #159. In addition, the fourteen (14) residents eating in the restorative dining room were not provided activities prior to the meal service. This practice hae the potential to affect more than a limited number of residents. Resident identifiers: #159, #13, #17, #135, #93, #86, #32, #124, #24, #63, #112, #15, #125, #90, and #30. Facility census: 99. Findings include: a) Resident #159 Resident #159 was interviewed on 07/21/15 at 10:55 a.m. after she requested to speak with a surveyor. She was intermittently tearful during this interview and reported she normally has a very active social life, but since being admitted to the facility she was extremely bored and had nothing to do except lay on the bed and watch television or walk in the halls. Random observations on 07/20/15 and 07/21/15 found Resident #159 crying in the hall of the facility adjacent to the nurses' station. On both occasions, she was observed being escorted back to her room and heard saying she was bored and wanted to go home. Review of the medical record on 07/21/15 at 2:30 p.m., revealed Resident #159 was admitted to the facility on [DATE], she was transferred to acute care on 05/13/15 and returned to the facility on [DATE]. Her [DIAGNOSES REDACTED]. Her admission minimum data set (MDS) assessment, with an assessment reference date (ARD) of 05/12/15, identified her activity preferences as having something to read such as books, paper and magazines, news and animals as somewhat important and music as very important. Resident #159's current care plan, marked as reviewed on 06/22/15, was silent in regards to activities for this resident and her activity preferences. Activity Director #198 reviewed the resident's care plan during an interview on 07/22/15 at 11:00 a.m., and confirmed she had not developed an activities care plan for Resident #159. b) Restorative dining room - Residents #13, #17, #135, #93, #86, #32, #124, #24, #63, #112, #15, #125, #90, and #30 1. On 07/20/15 at 12:00 p.m., an observation of the restorative dining room, found six (6) staff members standing near the sink and talking to each other. Staff did not interact with the fourteen (14) residents sitting at the tables, except when Resident #93 attempted to leave the room twice and when Resident #15 pulled the tablecloth off and backed away from his table. The television located at the end of the room away from the dining tables was on the Disney channel until the meal was served and no other activities were provided prior to lunch. 2. An observation of the noon meal service on 07/21/15 began at 11:51 a.m. Nurse Aides (NA) #52, #127, #124, #120, #171, and #221 were found standing near the sink and conversing while one (1) resident was in front of the television with the Disney channel on and thirteen (13) other residents were seated at their tables. At 12:07 p.m., the television was turned off and again no other activities were offered. At 12:10 p.m., Resident #15 crashed his wheelchair into the activities cart knocking games and puzzles onto the floor. He was redirected and offered coffee after he pulled additional games off the activity cart. At 12:20 p.m. Resident #93, left the table and started to exit the dining room. She was redirected and offered coffee. Lunch was served at 12:30 p.m. and Resident #93 left the restorative dining room without eating. NA #171 and #127 were interviewed on 07/21/15 at 12:29 p.m. They acknowledged there were no activities for the residents prior to lunch except the television with the Disney channel and agreed they should be interacting more with the residents to keep them occupied. The activities director was interviewed on 07/22/15 at 11:00 a.m. She reported the activities department did not provide activities for the residents that eat in this dining room prior to lunch. 2018-09-01
