In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The bed lock was not present/or functional on the bottom of the right side of the bed, allowing the bed to move freely. Resident Identifier: #165. Facility census: 100. Findings include: a) Resident #165 An observation of Resident #165's bed on 06/13/16 at 1:00 p.m., found the bed lock missing from the bottom right side of the resident's bed. The bed moved freely about. On 06/14/16 at 2:30 p.m., an observation of the bed lock revealed the brake remained missing from the bottom of the bed on the right side. The bed continued to moved about freely. During an observation and interview with the maintenance director on 06/15/16 at 10:15, he agreed the brake lock was missing on the right side of the bed.
