In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility failed to label and date food items in both the refrigerator and freezer areas, store items in a tightly sealed container and properly log refrigerator temperatures. The failed practice had the potential to affect more than a limited number of residents. Facility census: 76. Findings included: A policy review titled, Food Safety Requirements with no revised date noted, on 02/24/20 at 1:00 PM, revealed, Food will be stored, prepared and served in accordance with professional standards for food service safety. Refrigerator storage- Labeling, dating and monitoring food, including, but not limited to leftovers, so it is used by its use-by-date, or frozen (where applicable)/discarded. Keeping food covered or in tight containers. a) Initial Tour An observation during initial tour, on 02/23/20 at 10:35 AM, revealed an incomplete refrigerator temperature log located near door of the walk-in refrigerator. An interview with Assistant Kitchen Supervisor (AKS) #11, on 02/23/20 at 10:40 AM, confirmed on 0[DATE] and 02/23/20 refrigerator temperatures were not completed. AKS #11 stated that she would have expected that the dates of 0[DATE] and 02/23/20 to be completed by now. An observation in the refrigerator, on 02/23/20 at 10:45 AM revealed the following sanitation concerns: - One (1) pack of approximately 50 hotdog's were opened and not labeled or dated - Four (4) individual Dairy Pure low fat milk cartons laying on the floor of the refrigerator - A plastic bag that contained three (3) blocks of white American Cheese was not properly sealed and had no label or date. - A plastic bag that contained two (2) blocks of yellow American Cheese did not have a legible date. An immediate interview with AKS #11, on 02/23/20 at 10:45 AM, confirmed the hotdogs should have been labeled and dated, the white American Cheese should have been sealed tightly, labeled and dated, and the yellow American Cheese contained a purple faded smear where the date should have been located and was not legible. AKS #11 also confirmed the milk should not have been on the refrigerator floor and should have been picked up when dropped. An observation of the the outside freezer, on 02/23/20 at 10:55 AM, revealed two (2) large chunks of ice on the freezer floor. Upon further observation, there was a mound of ice that was built up around boxes stored in the back of freezer. A piece of paper was observed laying on the freezer floor. The paper was titled, Preparation instructions: For all Meats, Vegetables, Breakfast and Specialty Products. The paper was brown from dirt, torn and appeared to have been walked on several times. An immediate interview with AKS #11, on 02/23/20 at 10:55 AM, confirmed the two (2) large chunks of ice was residue from kitchen staff moving the boxes from underneath the mound of ice in the back of the freezer. AKS #11 stated, the ice build up in the freezer is a regular occurrence but kitchen staff has not made it out to the freezer lately to perform ice removal duties. AKS # 11, stated, the paper on the floor of the freezer must have been dropped by kitchen staff while in the freezer and no one picked it up from the freezer floor for a few days. b) Kitchen Follow-Up An observation during the kitchen follow-up, on 02/24/20 at 12:00 PM, revealed a free standing freezer located against the wall of the kitchen. The freezer contained the following: - Two (2) clear bags of six (6) cinnamon rolls which were not labeled or dated. - One (1) big clear plastic bag of 40 frozen biscuits which was not labeled or dated An interview with AKS #11, on 02/24/20 at 12:00 PM, confirmed she pulled the two (2) bags of cinnamon rolls from the outside freezer today and was going to use them today so did not label or date the bags. AKS #11 also confirmed the bag of frozen biscuits should have been labeled and dated. 2020-09-01
