In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

Resident identifier: #326. Facility census: 76. Findings included: a) Resident #326 Resident #327's Medication Administration Record (MAR) was reviewed after his medication pass was observed. Resident #327 was noted to have the following medication orders written on 02/12/20: - [MEDICATION NAME] 10 mg orally twice a day for hypertension, hold for systolic blood pressure less than 100 - [MEDICATION NAME] 50 mg every day, hold for systolic blood pressure less than 110 or pulse less than [AGE] He also had an order for [REDACTED]. Review of Resident #327 s Medication Administration Record (MAR) demonstrated he had received [MEDICATION NAME] every day at 9:00 AM and 9:00 PM. He had also received [MEDICATION NAME] every day at 9:00 AM. The resident's blood pressure and pulse measurements were not documented on the MAR. Additionally, the resident's blood pressure and pulse readings were not documented on the vital signs sheet. On 02/24/20 at 10:30 AM, the Director of Nursing (DON) stated blood pressure and pulse measurements for Resident #327 were recorded in the progress notes. Review of Resident #327's progress notes revealed blood pressure and pulse measurements had been recorded twice daily in the progress notes. On 0[DATE] at 8:00 AM, Resident #327's blood pressure was recorded as 98/[AGE]. Review of Resident #327's MAR documented he had received both [MEDICATION NAME] and [MEDICATION NAME] at 9:00 AM on 0[DATE]. On 02/24/20 at 10:45 AM, the Director of Nursing acknowledged Resident #327's blood pressure on the morning of 0[DATE] was below the parameters for which he should have received [MEDICATION NAME] and [MEDICATION NAME] administration. 2020-09-01
