In collaboration with The Seattle Times, Big Local News is providing full-text nursing home deficiencies from Centers for Medicare & Medicaid Services (CMS). These files contain the full narrative details of each nursing home deficiency cited regulators. The files include deficiencies from Standard Surveys (routine inspections) and from Complaint Surveys. Complete data begins January 2011 (although some earlier inspections do show up). Individual states are provides as CSV files. A very large (4.5GB) national file is also provided as a zipped archive. New data will be updated on a monthly basis. For additional documentation, please see the README.

The facility failed to check depression on the MDS section I Active Diagnosis. The failed practice affected one (1) of ten (10) residents. Resident identifier: #3. Facility census: 19. Findings included: a) Resident #3 An observation of Resident #3, on 04/15/19 at 11:00 AM, revealed Resident #3 was socially withdrawn from facility activities. An interview with Resident #3, on 04/15/19 at 11:00 AM, stated he felt tired all the time. Resident #3 stated that he participated in rehab but rehab wore him out so he doesn't participate in facility activities. A record review, on 04/16/19 at 09:40 AM revealed a current [DIAGNOSES REDACTED]. The minimum data set (MDS) dated for 02/27/19 revealed that depression was not marked as an active [DIAGNOSES REDACTED]. An interview with MDS Coordinator, on 04/16/19 at 10:00 AM, confirmed depression should have been marked on the MDS. MDS Coordinator stated I do not have depression marked, I missed it. 2020-09-01
