This failed practice had the potential to affect a limited number of residents. Resident identifier: #40. Facility census: 56. Findings included: a) Resident #40 Record review on 03/13/18 at 11:00 AM, found the resident was admitted to the facility on [DATE]. The residents first weight was recorded as 271.3 pounds on 10/06/17. A significant change in status MDS with an assessment reference date (ARD) of 02/01/18, noted the resident's current weight was 221 pounds. The MDS coded the resident as having no weight loss, (5% or more in the last month or loss of 10% or more in last 6 months.) An interview with the dietary manager (DM) #20, at 12:14 PM on 03/13/18, confirmed the resident had an actual weight loss of 18.54 % as calculated by dietary manager at the time of the MDS with an ARD of 02/01/18.
